Take Charge! Live Well! is an exciting new health and wellness program brought to you by the state's labor management Joint Health Care Committee (JHCC).

Keep it simple

While the program is entirely voluntary, it’s designed to be uncomplicated to encourage participation.

A good way to start the program is by filling out an optional confidential Health Risk Assessment (HRA) questionnaire that provides an easy-to-understand overview of participants’current and potential health risks.

The HRA is available online through any of the three health care providers or from a benefits administrator.

It takes about 20 minutes to complete.

After filling out an HRA, participants will receive $50 through payroll. Participants also may choose to earn incentives through signing up for additional health programs.
